摘要：与2014版WHO分类不同，咱们企图依据与病因学相关的形态学特征【即人乳头状瘤病毒（Human Papillomavirus，HPV感染】对宫颈腺癌（endocervical adenocarcinoma,ECA）进行分类。世界宫颈腺癌分类规范（International Endocervical Adenocarcinoma Criteria and Classification，IECC）经过高倍镜观察到的核分裂和凋亡小体区别人乳头状瘤病毒相关腺癌（human papillomavirus-associated adenocarcinoma,HPVA）和伴或不伴HPVA特征的非人乳头状瘤病毒相关腺癌（nonhuman papillomacirus-associated adenocarcinoma,NHPVA）。依据胞浆特征对HPVA进行进一步分类（首要与已有的分类坚持连续性）；而NHPVA则依据已树立的规范（即胃型、通明细胞等）进行再分类。从全球范围内的7家安排搜集409例病例的完好切片，构建包含297例病例的安排芯片。经过免疫安排化学（p16、p53、波形蛋白、孕激素受体）和能辨认18种高危HPV RNA显色的原位杂交证明了IECC的确诊。IECC的5种常见类型是普通型（HPVA）（73%），胃型（NHPVA）（10%），HPVA黏液型癌（9%，包含肠型、黏液型癌-非特指类型癌、印戒细胞型、浸润性复层发生黏液的癌），通明细胞型癌（NHPVA）（3%）和腺癌-非特指类型（2%）。其间仅有3例是子宫内膜样癌，且均为NHPVA。扫除安排处理欠安的要素，90%-95%的IECC普通型病例过表达p16，而且出现HPV+，而NHPVA别离有37%的p16+和3%的HPV+。复查时，1例HPV+胃型出现HPVA/NHPVA的混合特征。与HPVA肿瘤比较，NHPVA肿瘤体积更大且患者年纪较大（P＜0.001）。关于辨认HPV感染相关的普通型和黏液型癌，与p16的免疫安排化学组比较（别离为0.872、0.632、0.907、0.545），高危型HPV显色在原位杂交探针组具有较高的敏感性、特异性、阳性和阴性猜测值（别离为0.955、0.968、0.992、0.833）。IECC仅经过形态学可将ECA分为HPVA和NHPVA。本研讨证明，普通型ECA是最常见的类型，黏液型癌具有HPVA和NHPVA的混合特征，胃型癌是首要的NHPVA类型。子宫内膜样癌和浆液性癌极端稀有。若临床和基因组学的进一步研讨数据支撑上述内容，咱们引荐用2017版IECC规范替代2014版世界卫生安排规范进行分类。
